Minor Waivers

MINOR WAIVER POLICY

All minors must be accompanied by a parent or person showing legal proof or guardianship. The rider must have a minor waiver filled out by those parties upon EVERY VISIT to the facility. If the rider is coming in by themselves with a previously signed waiver, it must be notarized.

If the minor is accompanied by someone who IS NOT a legal guardian: Parents may write a letter giving another adult authorization to make medical decisions on behalf of the minor. This letter must be signed, dated, and notarized. NO exceptions.

-PRINT OUT THIS FORM-
-Have parent(s) of minor fill out the form completely
-Provide a separate letter, providing the name of the temporary guardian (must be 18 years or older), with written authorization that they are allowed to make medical decisions on behalf of the minor. This letter MUST be signed, dated, and notarized. NO EXCEPTIONS.
-Make sure both parents sign the minor waiver in the link above.
-Take both of these forms to a local Notary (seal and stamp required)  OR use an online notary such as notarize.com
-Email (connor@njmpfod.com) notarized online documents OR bring them (notarized waiver, letter of temporary guardianship, and a copy of the parent's drivers license(s) prior to registering for a ride pass.
