MINOR WAIVER RULES AND FORMS
Field of Dreams is officially sanctioned by the State of New Jersey and the AMA. All rules and policies will be strictly enforced. For any questions regarding minor rider regulations, please contact us before your rider's arrival at 856-765-3799 or 215-907-8566.
IF YOUR RIDER DOES NOT HAVE THE PROPER DOCUMENTATION THEY WILL NOT BE PERMITTED TO RIDE.
Minors Waiver Policy for Practice and Race Events
Per Sections 1.1, 2.1 and 3.1 of the AMA Rulebook, “No rider under the legal age of majority in the state in which the meet takes place may compete without the written consent (signature on liability releases and entry forms) of their parents or legal guardians present at the meet. An authorized adult acting on behalf of the parents or legal guardians must provide a notarized statement indicating they have been given the authority by the parent or legal guardian to be responsible for the minor during the meet. All releases and notarized statements must be forwarded to the AMA with the referee report and/or injury report. The parents, legal guardians or authorized adult must remain present while the AMA member who they are responsible for is at the meet.”

- If your notary needs a notary block to stamp and seal form #2, PRINT AND PRESENT THIS ACKNOWLEDGEMENT CERT and bring it along with both forms to your notary.
- Have both parents or Legal Guardians of minor(s) fill out the form completely. (Grandparents are not considered legal guardians unless appointed by a court order.)
- Be sure that the both parents (Legal Guardian(s)) signs this form.
- Take it to a Notary Public and have notarized (notary seal and stamp required for signature witnessing) If a notary isn't open for any reason, you may use an online notary such as notorize.com.
- Email or bring the notarized waiver, letter of Authorized Adult and a copy of the parents drivers license(s) prior to registering for a Ride Pass.
IF BOTH LEGAL GUARDIANS WILL NOT BE PRESENT -
TEMPORARY AUTHORIZED ADULT IS NEEDED
- Provide a separate LETTER TO BE COMPLETED stating the name of the AUTHORIZED ADULT (must be 25 years or older). This letter must be signed, dated and notarized by both legal guardians AND the Authorized Adult each time the rider/racer attends or you may annualize the Authorized Letter for the year. No exceptions.
- Be sure that both parents (Legal Guardian(s)) signs this form as well as the Temporary Authorized Adult.
- Take it to a Notary Public and have it notarized (notary seal and stamp required) If a notary isn't open for any reason, you may use an online notary such as notorize.com
- Email or bring the notarized waivers (above), letter of Temporary Authorized Adult and a copy of the legal guardians drivers license(s) prior to registering for a Ride Pass.
Pit Vehicle PROHIBITED
UNLESS AUTHORIZED BY MANAGEMENT.
